| Program Update |
OBJECTIVE REVIEW COMMITTEES/CAP SEPTEMBER 2001 Thanks to all who volunteered to be part of the review process for the new round of CAP applicants. The review period will take place between June 12- June 14 in the Washington, DC metropolitan area. All who expressed interest will be receiving a skills questionnaire within the next week to learn more about your interests and to place folks in groups for the review. Please send this questionnaire back as quickly as possible and you will receive more information on next steps to be part of the review. If you have not yet contacted us but are interested, please contact me as soon as possible.

EVALUATION REPORTING: Within the next few weeks, all 2000 grantees will be receiving six-month evaluation forms to complete and send back. These reports were created for your particular community based on your project matrix and logic model. These reporting forms are mandatory.

| TA Calls |
Technical Assistance Calls |
CAP continues to offer TA calls for grantees every other Tuesday from 2 to 3 or 3:30 PM EST. The schedule for July - August appears below. To register for the calls, please contact Angelique Raptakis at 301-468-6006 Ext. 501 or at araptakis@mac1988.com with your name and contact information at least one week prior to the call.
Shortly after you register, you will receive a call-in number and an access code. If you do not receive this call-in information by the Monday before the call, please contact Angelique. Agendas for TA calls will be emailed out ahead of time.

Mental Health and Substance Abuse Integration
This teleconference is designed for communities interested in developing
models and systems for integrating behavioral health and primary care
services. The goal of the conference is to examine the clinical, economic and
service quality rationales that support integration.

April 10 |
Overview of Legal Issues
An overview of the relevant state and Federal laws and regulations that pertain
to CAP. Will be hosted by the firms that hosted the HIPAA call. An agenda
and issue brief will be provided a week before the call.

April 24 |
Disease Management
At the end of the call, participants will know the benefits to incorporating disease management into a CAP program, how to practice disease management into a community setting, and how to co-ordinate with other disease management initiatives. Future sessions may focus on particular diseases such as asthma, hypertension, and diabetes.

While the site will continue to be developed, it currently has links to all grantees, information on peer-to-peer support by area, and a bulletin board to ask questions. You can also use it to ask TA questions and link to MAC to get assistance and consultant recommendations. New York City Health and Hospital Corporation |
New York City Health and Hospital Corporation has expanded its prenatal network partners to implement the national "Birthing Project" model in each of their respective service areas on a fully staffed/supplied level. They have also developed four regional interdisciplinary groups that include: community outreach, primary and prenatal care, and specialty, ancillary and acute care services. The goals of the CAP grant have encouraged the HHC partners to building integrated, bi-directional referral relationships as well as tracking and follow-up mechanisms to ensure proper access to care.

| UPCOMING CONFERENCES/GRANTS |
GRANT OPPORTUNITY: The American Academy of Pediatrics, through Reaching Children: Building Systems of Care, will be supporting local partnership initiatives that demonstrate innovative methods to improve outreach and enrollment of children nationwide. Each partnership initiative will be awarded $10,000 for a one-year period. UPCOMING CONFERENCE: On April 5 -6, 2001, in Asheville, NC - Consider attending the Project Access Community Event (PACE). "This special event will assist communities in achieving meaningful improvement in access to care using the experience of Buncombe County Medical Society Project Access as a model. PACE attendees will receive assistance in developing solid plans of action for identifying and aligning their resources to create community-based systems providing access to care and improved health for all. Your team will participate in interactive sessions with leaders from other communities who have been successful in creating true win-win situations. Everyone who attends will learn how these diverse communities effectively aligned existing community resources to create comprehensive 100% healthcare access systems capable of eliminating health disparities." On June 21-23, the Communities in Action conference will be held at the Marriott Wardman Park Hotel, Washington, D.C. The theme is Better Health for More People for Less Money. This is an important and exciting meeting to support communities in pursuing 100% Access and 0 Health Disparities. Partners of the Campaign for 100%/0 and communities interested in accelerating their pace of progress, learning, and networking with those that have demonstrated measurable outcomes will be in attendance and participating.
